Do I have everything I need for my appointment?

    • New Patients:  You’ll need your insurance card AND a picture I.D. Arrive 10-15 min early to fill out new patient forms.
    • New Insurance: Please bring your new insurance card or member I.D..
    • Please bring your current pair of glasses.
    • Double check if you are still eligible for your vision and benefits.
    • Has your insurance changed since my last eye exam? If so, please let us know!

Cancellation Policy: Please give at least 24 hours notice if you are unable to make your appointment. We’ll be happy to reschedule you!

Insurance

We accept most major forms of vision and health insurance including VSP, EyeMed, and United Health Care. Please see our full list of insurances we accept here.

Foreign Language Assistance

We have a staff member on site that is able to communicate in Spanish and American Sign Language (ASL) for patients on request. This is not an interpreting service but we’ll be happy to assist you during your entire exam!
